Medical Clearance Information

Medical Clearance Information

ALL student-athletes receive an email via ARMS during the first week of June detailing the medical clearance process. If you do NOT receive this email, please contact your head coach immediately for further direction. This contains vital information related to the documents and forms you will need to complete this process as well as a FAQ section. ALL printed forms must be uploaded directly into Teamworks AMS. Hard copies will NOT be accepted. Links to all forms can also be found below.

Medical Clearance Guidelines
  1. NEW student-athletes (freshmen or transfers) are REQUIRED to obtain a physical examination (using ONLY the Biola issued physical form linked above) from a personal physician. ALL physicals must be performed on or after March 1st of the current calendar year. Physicals performed before this date WILL NOT be accepted. (IF you are transferring in the Spring, contact the medical clearance coordinator for an updated date).
    • Exams must be performed by a licensed M.D., D.O., P.A-C., or N.P. If your physical exam is completed by a PA-C, it must be co-signed by their overseeing physician (MD or DO) to be accepted. Please ensure all medical providers sign the exam. Ensure their credentials are clearly visible (either via signature or office stamp). Exams completed by RN’s or Chiropractors (D.C.) WILL NOT be accepted.    
    • New student-athletes will also be required to attend Biola University Athletics’ annual physical event. This event WILL NOT serve as your personal physical examination. The purpose of this event is to receive final clearance for participation from our Team Physician.
       
  2. NEW student-athletes (freshmen or transfers) are REQUIRED to obtain their sickle cell test results. Please reference the email form ARMS or link above for details on how to obtain and upload those.
     
  3. RETURNING student-athletes are NOT required to obtain a new physical or sickle cell test. However, you are required to submit a Medical History Questionnaire each year. This form will be reviewed each year by Biola Athletic Training staff and, if deemed necessary, you may be referred to our team physician for further examination.
     
  4. ALL student-athletes are required to complete the informational forms and documents via Teamworks AMS.
     
  5. ALL student-athletes are required to either create an account (new student-athletes) or update their existing account (returning student-athletes) on Teamworks AMS.
     
  6. All student-athletes will be required to attend the medical clearance event appropriate for their team. See the medical clearance information email for details.

ALL FORMS MUST BE UPLOADED INTO ARMS NO LATER THAN JULY 31 for new SAs and JULY 15th for returning SAs or Men’s Water Polo. FORMS THAT ARE LATE WILL IMPACT YOUR ELIGIBILITY TO PARTICIPATE IN ANY TEAM ACTIVITY! 

Final clearance for participation will not be given until Biola University Athletics’ annual medical clearance event. If, for any reason, you are not cleared, you will be notified at this event.
