Get cleared

Every Biola student-athlete must be medically cleared before any team activity. Here is what you need, when it is due, and where it goes.

Watch your email in early June. Every student-athlete receives an email through Teamworks during the first week of June detailing the medical clearance process, the documents and forms you need, and a FAQ. If you did not receive it, contact your head coach immediately. All printed forms must be uploaded directly into Teamworks AMS; hard copies are not accepted.

Submission deadlines

DeadlinesJuly 24 for volleyball, soccer, women's basketball, men's water polo and cross country. July 31 for all other sports. All forms must be uploaded into Teamworks AMS by these dates. Late forms will affect your eligibility to participate in any team activity.

What you need to do

1 New student-athletes: physical examination

Freshmen and transfers must obtain a physical examination using only the Biola-issued physical form linked above. All physicals must be performed on or after March 1 of the current calendar year; earlier physicals are not accepted. If you are transferring in the spring semester, contact the medical clearance coordinator for an updated date.

Exams must be performed by a licensed M.D., D.O., PA-C or N.P. If completed by a PA-C, it must be co-signed by the overseeing physician. Make sure credentials are clearly visible by signature or office stamp. Exams completed by RNs or chiropractors are not accepted.

New student-athletes must also attend Biola Athletics' annual physical event. That event is not your personal physical examination; its purpose is final clearance from our team physician.

2 New student-athletes: sickle cell test

Obtain sickle cell test results. See the informational email or the instructions linked above for how to obtain and upload them.

3 Returning student-athletes: medical history questionnaire

Returning student-athletes do not need a new physical or sickle cell test, but must submit a medical history questionnaire each year. Biola athletic training staff review it, and if necessary you may be referred to our team physician for further examination.

4 Everyone: Teamworks AMS forms

Complete all informational forms and documents in Teamworks AMS.

5 Everyone: Teamworks AMS account

New student-athletes create an account. Returning student-athletes update their existing one.

6 Everyone: medical clearance event

Attend the medical clearance event appropriate for your team; see the medical clearance email for details. Final clearance to participate is not given until this event. If for any reason you are not cleared, you will be told there.
